Clinical Evaluation of Perioperative Infection Prophylaxis with Single Tiberal and Gentamicin Injection in Elective Gynecological Surgery



Abstract
The aim of this study was to evaluate the prophylactic effect of ornidazole(Tiberal) for prevention of infections in surgical patients in gynecologic fields.
Fifty patients who had to undergo abdominal surgery at the Department of Obstetrics & Gynecology, Seoul National University Hospital, from September 1, 1987 to December 31, 1987 were admitted to the study.
Each patient was randomly assigned to one of three groups with differing protocols of antibiotics.
Twenty patients in group I were intravenously administered with single 1000mg ornidazole in 500ml 5% dextrose in water and single 80mg gentamicin intramuscularly one hour before surgery.
Ten patients in group¥± were also given 1000mg ornidazole and 80mg gentamicin one hour before surgery, in the same way as those in group¥° followed by 3 further doses of 500mg ornidazole at 12 hours intervals and 3 doses of 80mg gentamicin at 8 hours interval postoperatively.
Twenty patients in group¥² were intravenously administered with 500mg metronidazole in 100ml solvent and 80mg gentamicin intramuscularly one hour befere surgery followed by 500mg metronidazole and 80mg gentamicin every 8 hours for 3 days postoperatively.
The results obtained were as follows;
1. Postoperative complications occurred in 1 case(5%) of group¥°; urinary tract infection and 1 case(5%) of group¥²; wound infection.
2. E. coli and enteroccocci were found in urine culture in 1 case of group¥° and no bacteria were found in wound culture in 1 case of group¥².
3. There were no significant differences between three groups in the incidence of complications.
